本人願意提供以上個人資料給福慧國際慈善基金作有關捐款及日後活動的聯絡通知。本機構將遵從香港特別行政區《個人資料(私隱)條例》的規定，把所有已收集的個人資料作保密處理並只供獲授權之工作人員使用。有關資料除指定負責人員外，將不會向外洩露或給予本機構以外的任何個人或團體。

 

I hereby agree to provide the above personal information to Fuhui International Charity Foundation for the purpose of donation and future event notifications. The organization will comply with the Personal Data (Privacy) Ordinance of the Hong Kong Special Administrative Region, keep all collected personal information confidential, and only use it by authorized staff. Except for the designated responsible personnel, the relevant information will not be disclosed to any individual or organization outside the organization.
